Vaccine Requirements
Based on the new Directive outlined by Ontario’s Chief Medical Officer, UHN has updated its COVID-19 policy to mandate that all UHN team members are fully vaccinated (internal connection required). This includes employees, physicians, graduate research students, research fellows, clinical learners, volunteers and contractors. Graduate students at U of T should also consult the School of Graduate Studies FAQ for more information on U of T's policies.
Proof of vaccination for both first and second doses must be submitted online to Health Services by October 22, 2021, even if you have already informed them of your vaccination status.
Notify Health Services of your proof of COVID-19 vaccination here. If you have problems accessing the Health Services site or submitting your proof of vaccination, please email Siff Zhang, Research People & Culture Analyst, at siff.zhang@uhn.ca.
